How the conditional logic works

This form shows or hides fields as people answer, so everyone sees only what matters to them.

  • “Who will cover for you?” appears only when “Type of leave” is “Annual leave”.
  • “Do you have a doctor’s note?” appears only when “Type of leave” is “Sick leave”.
  • “Due or adoption date” appears only when “Type of leave” is “Parental leave”.
  • “Reason” appears only when “Type of leave” is “Unpaid leave” or “Type of leave” is “Other”.

Is the leave request form free?
