How the conditional logic works
This form shows or hides fields as people answer, so everyone sees only what matters to them.
- “What went wrong?” appears only when “Your rating” is less than “3”.
- “Would you like us to contact you?” appears only when “Your rating” is less than “3”.
- “Your email” appears only when “Your rating” is less than “3” and “Would you like us to contact you?” is “Yes”.
- “A link to a photo (optional)” appears only when “Your rating” is more than “3”.
