How the conditional logic works

This form shows or hides fields as people answer, so everyone sees only what matters to them.

  • “What would help you feel more valued?” appears only when “Do you feel valued at work?” is less than “3”.
  • “What one thing would change your score?” appears only when “How likely are you to recommend us as a place to work?” is less than “7”.
  • “How should we reach you?” appears only when “Would you like someone to follow up?” is “Yes”.

Is the employee engagement survey form free?
