How the conditional logic works

This form shows or hides fields as people answer, so everyone sees only what matters to them.

  • “How many people?” appears only when “Type of membership” is “Family”.
  • “Monthly gift” appears only when “Would you like to give regularly?” is “Yes”.

Is the membership form form free?
