How the conditional logic works

This form shows or hides fields as people answer, so everyone sees only what matters to them.

  • “When did you lose it?” appears only when “Type of request” is “A replacement for a lost key”.
  • “Return date” appears only when “Type of request” is “A temporary key”.
  • “Manager who approves” appears only when “Which door?” is “Server room”.
